Job Description:
A company is looking for a Social Services Care Manager,
Behavioral Health.
Key Responsibilities
Assess and evaluate members' needs related to physical,
environmental, and psycho-social health
Facilitate care coordination and linkage to community resources for
members and caregivers
Create and update member care plans and maintain accurate
documentation of outreach and contacts
Required Qualifications
Master's degree in social work from an accredited university
Current, unencumbered social work license (LMSW, LCSW, LICSW)
Passed Master, Advanced Generalist, or Clinical ASWB Exam
Minimum 3 years of experience as a social worker in a healthcare
setting
Proficient in Microsoft applications including Word, Outlook, and
Excel
