About the Role:We are seeking a skilled Home Health Physical Therapist to join our team at AccentCare. As a Home Health Physical Therapist, you will be responsible for providing high-quality patient care in the comfort of their own homes.Key Responsibilities:

• Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to improve their mobility and function.
• Provide one-on-one therapy sessions to patients in their homes, focusing on exercises and activities to promote independence and overall well-being.
• Collaborate with other healthcare professionals, such as nurses and occupational therapists, to ensure comprehensive care and coordination.
• Communicate effectively with patients, families, and caregivers to educate them on treatment plans and progress.
• Document patient progress and maintain accurate records of treatment plans and outcomes.Requirements:
  • Current licensure as a physical therapist in the state of operation.
  • Minimum 9 months of experience as a physical therapist.
  • Preferred experience in home health or community settings.
  • Master's degree in physical therapy from a CAPTE-accredited program.
  • Valid CPR certification.
  • Licensed driver with reliable transportation.What We Offer:
