Job Description:
A company is looking for a Health Coach - Diabetes.
Key Responsibilities
Assist members by completing health assessments and implementing
care plans for chronic conditions
Provide telephonic support and coaching to improve members' health
and lifestyle
Monitor wellness plans and engage with healthcare providers to
align care plans
Required Qualifications
Associate's degree with 2 - 4 years of related experience
Certification as a Registered Dietitian, Physical Therapist,
Respiratory Therapist, RN, or Master's in Behavioral Health
Health Coach Certification is required
Residency in Oklahoma is required
CDCES certification is preferred
