Job Description:
A company is looking for a Transition Specialist to support the
coordination of member transitions in the community.
Key Responsibilities
Support care coordination teams to develop effective transition
plans for members
Assist with the transition process for members based on enrollment
or care service changes
Track and maintain transition metrics to identify trends and
process improvements
Required Qualifications
Bachelor's degree required
2 - 4 years of related experience preferred
Equivalent experience may be considered
Knowledge of healthcare services and transition planning
Experience in developing educational materials and training
programs
